As a service to the public, Lakewood wishes to encourage the dissemination of GIS information to you as inexpensively as possible.

Download and complete the Standard Map Product form (pdf 40 kB). Mail to the City of Lakewood along with total purchase/shipping costs. Costs vary depending on delivery, media type and quantity. Please include a written request detailing the project that the data will be used for and a signed/dated disclaimer form (pdf 18 kB). Requests will be filled when monies are received by the City of Lakewood.
